Information to include with Reach issues

To help us quickly troubleshoot Reach issues, we need the following information:
  1. Username for the account experiencing issues
  2. Project ID/name
  3. Template ID/name
  4. Are you using the Dashboard, API, or a combination of both?
    1. If you are using the API, please include the full request and full response including headers
  5. Steps to reproduce the issue
  6. Screenshots of errors, if applicable
